Do All ACL Injuries Need Surgery? - Part 1

Misconception: “An ACL Injury Means I Need Surgery”

Why surgery isn’t always the only path—and how personalised rehab might be all you need.

If you’ve recently suffered an ACL injury—or know someone who has—you’ve probably heard the same advice: “You’ll need surgery.”

It’s a common assumption, but it’s not always true. At RHP, we work with many people recovering from ACL injuries. And what we see every week—and what the research supports—is that ACL rehab can be highly successful without surgery, depending on your goals, age, and activity demands.

The Evidence-Based Reality of ACL Recovery

If you're over 45 and not planning a return to high-level cutting, pivoting, or jumping sports, surgery might not be necessary. In this age group, ACL tears often occur alongside other changes in the knee like early osteoarthritis. For these individuals, a well-structured rehab program may restore enough function for daily life and low-impact activities.

Recent studies have shown that some ACLs can spontaneously heal or develop scar tissue that restores enough knee stability. This allows a return to functional movement, even without full anatomical repair. This is particularly true when rehab focuses on neuromuscular control, balance, and lower limb strength.

Believe it or not, up to two-thirds of athletes achieve a successful return to sport with non-surgical ACL management. Success is driven by a high-quality rehab program, individualised load management, and a progressive return-to-sport plan.

Surgery vs Rehab: It’s Not Black and White

Multiple long-term studies show similar outcomes in function, pain, and satisfaction between people who had ACL surgery and those who went through structured rehab. So what’s the right choice? It depends on:

  • Your age
  • Your activity goals
  • The function of your knee
  • And your response to initial rehab

At our clinic, we help you explore your options without pressure—and with clarity.
